EHR (Electronic Health Record)
A digital version of a patient's complete medical history maintained by a healthcare provider or health system.
Electronic Health Record (EHR) is a comprehensive digital repository of a patient's medical history, maintained by a healthcare provider, hospital, or health system. Unlike paper charts or older EMR systems, EHRs are designed for interoperability — allowing multiple providers within a health system to access and update shared patient information in real time.
What EHRs Contain
- Clinical notes from office visits and procedures
- Medication lists and prescription history
- Lab results and imaging reports
- Immunization records
- Allergy information and contraindications
- Insurance and billing information
AI Voice Agents and EHR Integration
When a patient calls an AI voice agent for appointment scheduling or test result delivery, the agent queries the EHR to confirm the patient's identity, check their medication allergies, retrieve recent test results, and update the record with call disposition. This integration requires HIPAA-compliant API design and data encryption.
HIPAA and EHR
EHRs contain Protected Health Information (PHI). Any voice platform accessing an EHR must have a BAA in place, encrypt all data in transit, and audit all access logs.
